	You may choose to book your place now on one of the residential dates listed below. However, please note this is accepted as a firm booking subject to our terms and conditions. If you wish to leave the date unspecified, you may leave the options unticked but we cannot guarantee availability if booking is left too late.

Full Payment
Option 1:	I enclose one cheque as full payment
Option 2:	Please debit my credit card for the full fee. I have completed the credit card authority enclosed / telephoned you with my credit card details and authority
Payment by Instalments
Option 1:	I have paid/enclosed my deposit and would like to pay the balance by installments. Please find enclosed a completed credit card/standing order mandate which authorises payments to commence now.
Payment by Career Development Loan
Option 1:	I have *paid/enclosed my deposit and am applying for a CDL for the balance. I have also enclosed the CDL declaration stating that I understand that should the CDL not be approved payment is due from myself.
Deferring Course Commencement
Option 1:	I have paid/enclosed my deposit together with (delete as appropriate) a post-dated cheque * a completed credit card authority with payments commencing at my chosen course start date * a completed standing order authority with payments commencing at my chosen course start date.
